• Form 1 – Registration Form
• Form 2 - Sponsor Certificate The Sponsor Certificate is to be completed once the Sponsor is known and forwarded to the Confirmation Coordinator.
• Form 3 - Confirmation Agreement - Outlines the Candidate’s obligations to the program.
• Form 5 – Service Completion Form
  The Service Completion Form is to be completed by the Candidate as well as the supervisor of the service work and returned to the Confirmation Coordinator. A list of opportunities will be posted on the web site shortly.
